This Privacy Policy ("Policy") governs the collection, processing, storage, use, disclosure, and transfer of information by Forme ("Company," "we," "us," or "our") in connection with your access to and use of the Forme mobile application and associated services (collectively, the "Service"). By accessing or using the Service, you acknowledge and consent to the practices described herein. If you do not consent to this Policy, you must immediately discontinue use of the Service.
THIS SERVICE IS INTENDED SOLELY FOR INDIVIDUALS EIGHTEEN (18) YEARS OF AGE OR OLDER. WE DO NOT KNOWINGLY COLLECT INFORMATION FROM PERSONS UNDER THE AGE OF EIGHTEEN (18).
We may collect, receive, generate, and process the following categories of information in connection with your use of the Service:
(a) Identifying and Registration Information: Information voluntarily provided by you during account creation or onboarding, including but not limited to name, pseudonymous username, and electronic mail address.
(b) Transactional and Behavioral Data: Records of interactions, inputs, submissions, and exchanges occurring within the Service, including but not limited to session-level communications, user-generated content, response patterns, engagement metrics, and longitudinal usage data derived from your ongoing interaction with the Service's features.
(c) Technical and Device Information: Anonymized or pseudonymized device identifiers, operating system data, and authentication tokens generated for the purpose of service integrity and security verification.
(d) Inferred and Derived Data: Information derived or inferred from the foregoing categories through automated processing, algorithmic analysis, or pattern recognition conducted in furtherance of service personalization and delivery.
The Company processes the information described herein for the following purposes: (i) provision, maintenance, and improvement of the Service; (ii) personalization of Service features and content based on user characteristics and behavioral patterns; (iii) authentication, fraud prevention, and security monitoring; (iv) fulfillment of contractual obligations including Subscription management and billing facilitation; (v) compliance with applicable legal obligations; and (vi) communication with users regarding Service-related matters, subject to applicable consent requirements.
Certain categories of information are stored locally on your device pursuant to standard mobile application data persistence mechanisms. Additional data may be transmitted to and stored within secured cloud-based infrastructure operated by or on behalf of the Company or its authorized processors. The Company employs reasonable technical and organizational measures designed to protect information against unauthorized access, disclosure, alteration, or destruction. Notwithstanding the foregoing, no method of electronic transmission or storage is completely secure, and the Company cannot guarantee absolute security of your information.
The Company does not sell personal information to third parties for monetary consideration. The Company may disclose information to trusted third-party service providers, vendors, processors, and infrastructure partners engaged to perform functions on the Company's behalf, including but not limited to authentication services, cloud data storage and processing, artificial intelligence and machine learning inference infrastructure, subscription and payment processing platforms, and analytics providers. Such disclosures are made pursuant to written data processing agreements that impose confidentiality and security obligations consistent with applicable law. The Company may also disclose information: (i) as required by applicable law, regulation, or legal process; (ii) to enforce this Policy or the Terms of Service; (iii) to protect the rights, property, or safety of the Company, its users, or the public; or (iv) in connection with a merger, acquisition, or other corporate transaction, subject to appropriate confidentiality protections.
THE SERVICE IS STRICTLY PROHIBITED FOR USE BY INDIVIDUALS UNDER THE AGE OF EIGHTEEN (18). The Company does not knowingly solicit, collect, or process personal information from any individual under the age of eighteen (18). In the event the Company becomes aware that information has been inadvertently collected from a person under the age of eighteen (18), the Company will take prompt steps to delete such information from its systems.
Subject to applicable law, you may have certain rights with respect to your personal information, including rights of access, correction, deletion, restriction of processing, and data portability. To submit a data request or to request deletion of your account and associated information, please contact the Company at the address set forth below. The Company will respond to verifiable requests within a reasonable timeframe consistent with applicable legal requirements.
The Company reserves the right to modify this Policy at any time. Material changes will be communicated through the Service or by other reasonable means prior to taking effect. Your continued use of the Service following notice of any modification constitutes your acceptance of the revised Policy.
